Coordinated Care at Clermont Park
Clermont Park offers residents friendship, freedom, and opportunity. At our senior living community in Denver, we are always looking for new ways to help residents and their families. Beginning in 2025, we are partnering with Curana Health for an in-community healthcare clinic available to all residents. Curana works specifically with senior living communities across the country to provide primary care and coordination. We also have our own Medicare Advantage plan created just for our residents, Perennial Advantage. Perennial Advantage offers a variety of plans built for each living option found at our community.

Perennial Advantage
Perennial is a customized Medicare Advantage Plan partially owned by our parent-company, Christian Living Communities, for the sole purpose of serving residents’ specific needs. Their mission is to provide better outcomes, increased satisfaction, and lower health costs to our residents. It all starts by bringing attention to the preventive lifestyle and chronic care management within our community. Curana Health On-Site Clinic
Curana Health gives Clermont Park residents the convenience of physician-led care teams right down the hall in the Curana on-site clinic or in the privacy of their home or suite. With a dedicated nurse practitioner and clinical concierge, the clinic supports older adults with regular checkups, manages symptoms of illness, and coordinates care through the continuum of services on campus, including consistent and predictable on-site rounding. With established clinic hours, the team can also support off-site medical visits, secure lab results, and advise on preventive care.
This trio of partnerships sets Clermont Park apart from many other senior living communities, as we offer coordinated care with the individual health of each older adult of paramount importance to us!
Perennial Advantage Plans
Unlike traditional Medicare Advantage plans, Perennial offers continual enrollment due to the special nature of each plan.
Freedom (HMO)
The Freedom Plan is available to residents who have Medicare A & B, and who are living in a Residential/Independent Living or Assisted Living neighborhood.

Strive (HMO I-SNP)
The Strive plan is just for residents who have Medicare A & B and Medicaid, and who are living in a Skilled Nursing neighborhood or living in an Assisted Living neighborhood and need assistance with two or more activities of daily living.
